The vision of Community Data Exchange Collaborative (CDEC) is to create a scalable closed loop care coordination platform that communities can leverage regardless of size or resources. Through a shared infrastructure and comprehensive onboarding model, we help communities start small, confirm value, and grow their care coordination activities when ready. Our platform provides the key components to a closed loop referral platform including the technical infrastructure, data use agreements, client authorization processes, training programs, and privacy and security policies. In sharing this core infrastructure, CDEC seeks to lower costs, strengthen collaboration, and elevate outcomes for the people and communities we serve.
