What is a CIE?

A Community Information Exchange (CIE) is an ecosystem comprised of
multidisciplinary network partners that use a shared language, a resource database,
and an integrated technology platform to deliver enhanced community care
planning. Care planning tools enable partners to integrate data from multiple
sources and make bi-directional referrals to create a shared longitudinal record.

Services
Data Exchange Infrastructure

The CDEC provides centralized functionality for your team. We provide the legal and technology framework that can be customized to your local community needs for care management, directory of local services, and prioritized use cases or strategic partnerships.

Consultation

CDEC staff offer customized 1:1 consultation service for communities: planning and governance work, selecting a technology platform, integration/data feeds, tailored report development, use case selection and strategy.

Technical Assistance

System administrators at CDEC are available to support 1:1 troubleshooting, system management, and reporting.

Resource Database

A standardized listing of health, human, and social services providers service offerings, eligibility, and intake information to efficiently match individuals with appropriate services.

Integrated Technology

A technology platform facilitates the integration of individual data from multiple partners' data systems to populate an easy-to-access single. longitudinal person record, enables bidirectional closed-loop referrals, and care coordination alerts.

Cross Sytem Care Planning

A CIE enables a proactive system of care enabling multidisciplinary partners to work collectively and contributing to a single care plan and identify system interaction.

Shared Language

The use of a unified individual longitudinal record reflects shared language defining needs, risk assessments, care planning, and individual outcomes.

What To Expect As A CDEC Partner

  1. Initial Engagement: A meeting that provides an overview of the VCCIE’s purpose and commitment to the community and begins to gather information on your organization and network. 
  2. Participation Agreement: The Participation Agreement governs the access and protection of client data. The Participation Agreement will be signed by all onboarding organizations.
  3. Organizational Setup Assessment: Participants complete a set of questions to determine the organizational and user set up for the system.  3b: Integration Discovery: Organizations who are interested in connecting their client management system to VCCIE can meet to learn more about the integration options.
  4. Training Program:  A training will be offered to support partner organization’s integration of the VCCIE’s universal client authorization form/language into the intake processes. The training will also include a demo of the VCCIE system, including its role based access design and how participants receive approved access based on their credentials.
  5. Go Live: Participants who have completed the onboarding have full access to the VCCIE.

Community Partner Group Funding Opportunity

We’re excited to announce that the VCCIE is currently taking applications to fund a second cohort of Community Partner Groups! Ventura County based 501c3 social service providers who are committed to improving care coordination throughout the County are encouraged to apply.  For a successful application the following criteria need to be met:  

1) Operate as a non-profit Ventura Based 501c3 organization

2) Attend an Orientation session 

3) Attend an Engagement meeting (or listen to the recording)  

4) Submit an OPIS form as the first step to start the Participation Agreement process
