What is a CIE?

A Community Information Exchange (CIE) is an ecosystem comprised of
multidisciplinary network partners that use a shared language, a resource database,
and an integrated technology platform to deliver enhanced community care
planning. Care planning tools enable partners to integrate data from multiple
sources and make bi-directional referrals to create a shared longitudinal record.

Community Information Exchanges  share common technical and operational components. The CDEC leverages its experience and infrastructure to share these components with partners who seek to reduce their costs.  The following outlines each component and the associated services offerred.

Legal Frameworks

Responsible exchange of client information requires a rigorous legal framework that protects the clients, the participating organizations, and the governance structure facilitating the process. The creation of the legal framework requires a harmonization of local, state, and federal privacy and security regulations and the operational realities to implement these regulations in a user-friendly way. CDEC has spent hundreds of hours developing participation agreements, client consent processes and security policies and protocols. Joining the CDEC provides an opportunity to leverage crucial infrastructure saving time and money.

Technical Infrastructure​

Leveraging Salesforce as the software and Boomi as the middleware, CDEC offers a full service closed loop referral platform including provider and client portals. Participating organizations are able to manage referrals, search local resources ( as applicable) and track utilization through a myriad of reports. Clients are also able to access their referral information directly thereby giving them more agency over their care. In addition, CDEC offers a full set of integration services ranging from simple SFTP file transfers to fully integrated API connections. The platform is HIPAA compliant and monitored by ICE Cyber security team ​

Onboarding a diverse set of community organizations to one single platform ( either through integration or provider portals) is a very complex process. CDEC has developed a systematic approach to onboarding organizations to the platform including contracting, training and monitoring. With a fully documented onboarding process, CDEC leverages customized Salesforce CRM functionality with automations to support outreach, contracting, onboarding, and reporting. ​

Network Management

A key component to a Community Information Exchange is a resource directory that reflects available services in the area. There are typically organizations ( such as 211 or other resource groups) that develop these directories locally. CDEC platform allows for a real time API feed of existing resource directories to populate the CDEC platform. CDEC partners then build on these directories through their outreach and referral management activities that are supported by the CDEC system. Partners are able to track onboarded organizations’ level of engagement and referral activity to support expedited referral response times and engagement of network members.​

Local Governance

Successful community information exchanges are a reflection of the local communities they seek to serve. Governance structures that provide insight into local challenges, priorities and opportunities can help ensure alignment for successful implementation. CDEC has experience in supporting local governance structures as needed and/or implementing pilot projects / use cases under the direction of the local stakeholder groups. Leveraging the CDEC infrastructure enables partners to spend less time on the core legal, technical and policy activities and more time on building support, alignment and adoption within the local communities. ​

For some communities a broad community infrastructure is not needed. It may be that providing a targeted care coordination resources between a small subset of organizations would represent a significant impact for the community.

The CDEC provides centralized functionality for your team. We provide the legal and technology framework that can be customized to your local community needs for care management, directory of local services, and prioritized use cases or strategic partnerships.

Consultation

CDEC staff offer customized 1:1 consultation service for communities: planning and governance work, selecting a technology platform, integration/data feeds, tailored report development, use case selection and strategy.
